How can I access my records or my child’s records?
You may contact your clinician to request access to records. A consent or records request form may be required. Records can often be shared electronically through the client portal. Printed copies may include a fee to cover administrative time, paper, and printing costs.
Are therapy records the same as psychotherapy notes?
No. Therapy records and psychotherapy notes are not always the same. Some documentation may be part of the clinical record, while psychotherapy notes may have additional protections. Your clinician can explain what records may be available and what steps are required to request them.
Is therapy confidential?
Therapy is confidential with certain legal and ethical exceptions. Limits to confidentiality may include safety concerns, suspected abuse or neglect, court orders, or other situations required by law. Confidentiality and its limits are reviewed during the intake process.
